How it’s handled

Form submissions are delivered to our team by email through a secure form-delivery service (Web3Forms). They are not stored in a database on this website. Please don’t send medical details, insurance numbers, or Social Security numbers through the forms. We’ll collect anything like that privately by phone or in person.

Third-party services

This site loads fonts from Google Fonts and is hosted by Cloudflare. These providers may receive basic technical information, such as your IP address, when you visit.

Your choices

You can ask us to update or delete the information you sent by calling (209) 808-9265 or emailing [email protected]. California residents may have additional rights under state law, and we will honor those requests.
